Responsible To : Director of Assisted Living
Summary of Job:
Responsible for assuring the delivery of health care and services to Lodge and Oak Arbor to ensure the implementation of residents’ care plans.
Responsibilities:
- Supervise Resident Assistants (RA) and monitor private duty nurses/aides and notify their agency of any pending problems.
- Provide training and instructions to RA’s as needed.
- Assess, plan, implement, and evaluate resident’s care plans
- Inform resident care team (MD’s) of any change in resident condition.
- Act as a liaison between residents and doctors in order to present residents’ needs to doctor for accurate care.
- Inform residents’ family of acute problems and potential and actual transfers to a higher level of care.
- Process medication and treatment orders from physician. Implement new orders and communicate to team in order to assure continuity of care and accurate records
- Keep Director, Asst Director and Clinical Coordinator informed of any changes in residents and/or needs that require Magnolia Grove (MG) team or Rehab Dept. evaluations. Follow through on necessary implementation.
- Work with Clinic and/or (MG) personnel to provide continuity of care for residents as deemed necessary.
- Complete admit / discharge / transfer documentation per policy.
- Assist residents with medication and do treatments (within scope of practice).
- Order medications and medical supplies and assure both are properly stored
- Schedule medical appointments, lab work, hearing checks, hospital tests or outpatient surgeries as necessary
- Report all incidents and accidents involving staff and/or residents to Director. Complete proper paperwork within 24 hours.
- Provide input for Resident Assistant performance appraisals.
- Evaluate requests for housekeeping and repair needs prior to reporting to OTL Admin.
- Assist and educate family members to understand policies and procedures at the Lodge. (SUP)
- Attend staff, supervisor and other meetings.
- Attend mandatory in-services.
- Provide instructions to RA’s and private duty nurses/aides as necessary.
- Practice safe work habits.
- Perform other duties as assigned.
Qualifications:
- Current license by the State of California as a Licensed Vocational Nurse.
- Two (2) years of experience as an LVN preferred.
- Knowledge and experience of working with geriatric population.
- Ability to work harmoniously with residents and staff and promote teamwork.
- Remain responsible and calm in an emergency situation, with ability to set priorities.
- Ability to meet physical requirements of the position.
- Possess a current CPR and first aid certificate.








Jay Labinger is a Gardens resident, he was born and raised in Los Angeles, CA. Jay received his Bachelor of Science in Chemistry from Harvey Mudd College. He attended Harvard University where he earned a Ph.D. He was an Assistant Professor of Chemistry at the University of Notre Dame. His career has included positions with Occidental Research Corporation & Atlantic Richfield. He is the Administrator of Beckman Institute at the California Institute of Technology as well as a Faculty Associate in the Chemistry Department. He enjoys reading, playing the piano, tennis and traveling. He has published books on the history of chemistry, sociology of science and literature & science. Here at the Gardens, Jay is part of the Conservation & Sustainability Committee the Green Fund as well as many other groups through the Gardens Club.
Jane Hurd is a Gardens resident, she was born & raised in a small town in Arkansas. After she received her BA from the University of Arkansas she moved to California and taught seventh grade English in Porterville CA. After two years she moved to Los Angeles and worked for the Department of Health Services and pursued a master’s degree in public administration from USC.  She served as CEO for two Los Angeles area hospitals. She continues to consult as an executive coach for CEOs of healthcare companies.  She has served on several boards including Mutual of America Insurance Company, YMCA of Metropolitan LA, LA Visiting Nurses Association and Inland Empire Girl Scouts. Jane is an award-winning author and plays several musical instruments. Here at the Gardens, Jane is part of the newly formed Bequest Committee, as well as many other groups through the Gardens Club.
