SOCIAL WORKER
Full time position in Magnolia Grove | Pay range: $32.00- $38.00 | 2 years college required
Responsible To : Director of Nursing Services
Summary of Job:
Under the direct supervision of the Director of Nursing Services, the Social Worker (SW) will provide necessary social services support to residents and their families in the skilled nursing facility. Acts as an advocate staff member to residents and their families to assure quality resident-directed care. Is responsible for the overall social service treatment plan, MDS, CAA’S, and Care Plan in the medical record and its implementation.
Responsibilities:

- Assures resident’s needs are being met (i.e. hair appointments, hearing aids, glasses, personal furniture and effects, advance directives, POLST, etc.).
- Completes necessary documentation in a timely manner.
- Attends Continuity of Care, Quality Assurance and Assessment Committee, Inter-disciplinary Team, Care Plan and other meetings as requested by supervisor.
- Conducts Social Service audits to be presented at Quality Assurance and Assessment meetings.
- Consults with consultant LCSW on a monthly basis and documents these meetings in consultant file.
- Insures discharge planning is initiated and implemented according to department and facility policy and procedures.
- Provides room visits and supportive active listening to residents and family members in the skilled nursing facility that are upset or disoriented.
- Provides staff liaison with Family Council meetings (if active) for family/friends of residents in the skilled nursing facility and acts as advocate for residents.
- Acts as advocate with State Ombudsman to assure resident rights.
- Acts as liaison with family members and other staff/administration regarding reporting of lost property.
- Provides residents and families with community referrals as deemed appropriate.
- Works with Activity Coordinators as needed.
- Schedules podiatry and dental appointments for residents as needed.
- Ensures updating and maintaining of the Social Services Policy and Procedure Manual.
- Responsible for weekly care plan schedule and provides coordination of the meetings and phone conferences, with the Resident Care/MDS Coordinator.
- Advocates and implements culture change for resident-directed care with Inter-disciplinary Team.
- Completes other duties as assigned by Director of Nursing Services.
- Practices good safety habits.
Qualifications:
- Social Service Designee certification required
- High School diploma – two years college required.
- Ability to follow oral and written directions well.
- Ability to deal effectively with residents, family members, staff, volunteers and service providers.
- Ability to exhibit compassion and patience for the elderly.
- Ability to work flexibly with others.
- Valid California driver’s license.
- Social Services experience is preferred.
- Knowledge and ability to use the computer correctly.
- Advocate of organizational values to support resident-directed care.
